Coordinated Access

Coordinated Access (CA) is a system between service providers, and government agencies that help people move from homelessness into transitional or permanent housing.

The HSD has a centralized database that tracks applicants and available housing. Furthermore, this database prioritizes access to housing resources based on individual needs and assessments.

Learn about the Multnomah Services & Screening Tool (CA Redesign)

Launched in October 2024, the MSST is a simpler version of the previous tool and features trauma-informed questions. Additionally, it aligns with local priorities and promotes equitable access to housing resources.

How do I connect to a Housing Specialist to get assessed?

For Adults: Call the Coordinated Housing Access Team (CHAT) Hotline at 844-765-9384.

Complete an online pre-screening by filling out this form.

Culturally-specific assessments for BIPOC people are available via the BIPOC CHAT Line at 503-280-2600 x 654 or housingreferrals@ulpdx.org.

For Veterans: Call the services hotline at 855-425-5544.

For Families: Reach out to 211 assessment.
