Coordinated Access

Coordinated Access helps people experiencing homelessness find housing through problem-solving conversations, connection to local resources, and matches prioritized households to Supportive Housing programs in Multnomah County.

Rather than going to every organization with resources, a household seeking services completes a single assessment: the Multnomah Services and Screening Tool (MSST).

The MSST prioritizes eligible households for case management and coordinates referrals into Supportive Housing programs that meet households’ needs and preferences.

How does Coordinated Access work?

Consent to Release of Information

If prioritized, households will be assigned a case manager to:

Help reduce barriers to housing

Support to obtain important documents

Connect households to Culturally-Specific services

Complete a warm hand-off to a housing program

TH programs help residents gain stability to obtain their own housing

RRH programs taper down assistance until clients take over their own rent

PSH programs support our most vulnerable neighbors to age in place or transition to assisted living programs

The Move-On program provides rental subsidy and light touch support for households no longer needing intensive services

How do I request a MSST?

Adult only households:

  • Attend Coordinated Housing Access Team (CHAT) drop in hours
  • -or- complete an online MSST pre-screening form
  • -or- call the CHAT warmline: 844-765-9384

Culturally-Specific assessments for the BIPOC community:

  • Attend BIPOC CHAT drop in hours
  • -or- call the BIPOC CHAT warmline: 503-280-2600 (Option 3)
  • -or- email: housingreferrals@ulpdx.org.

Families with minor children:

  • Call 211

Veterans, Survivors of Domestic and Sexual Violence, and Youth under age 25 may also be eligible to complete the MSST.
