Grievances & Reasonable Accommodation Requests
Grievances Against Provider or Partner Agencies
A grievance is defined by HSD as a documented, formal complaint that you have been: treated unfairly, denied services without explanation, or experienced harassment or discrimination while accessing or attempting to access HSD-funded services.
If you need to submit a grievance about a specific provider or partner agency, you must first contact that agency directly. Each provider or partner agency is required to have a grievance process in place that they must make readily available and accessible when requested.
If your grievance is rejected or denied, the next step is to schedule an appointment with the Director or Assistant Director of that provider or partner agency.
Grievances Regarding Unsatisfactory Results
If you have already contacted the provider or partner agency about your grievance directly and still haven’t found a solution, we may then refer your grievance to the appropriate team member in our office. We appreciate your patience as the grievance process can take time.

Reasonable Accommodations
Please use the following form to submit reasonable accommodation requests directly to the Homeless Services Department for HSD-specific services, programs or events. Services, programs, or events may include but are not limited to the coordinated access program, community engagement meetings, and HSD-led conferences.
For in person events please submit an accommodation request at least one week prior to the event.
If you need to submit a reasonable accommodation with a provider, you must contact them directly and submit an accommodation through their organization.
