Health Insurance
Medicaid — Oregon Health Plan (OHP) Eligibility
There are several pathways to apply for the Oregon Health Plan, Oregon Medicaid program. These resources can also help low-income individuals discuss state programs that help individuals pay for Medicare costs.

- Older adults and people with disabilities should call the Aging and Disability Resource Connection (ADRC).
- Others may apply for benefits by working with an assistor who can help answer questions and get applications approved more quickly. Use this tool to locate an Oregon Health Plan Assistor or contact Multnomah County Eligibility Team (OHP Community Partner). Some housing programs have staff who are trained as assistors.
- Individuals can also apply by visiting a a local ODHS office or Call ONE Customer Service

Medicaid — OHP Ombuds Office
The Oregon Health Authority has a team of staff called Ombudspersons, whose goal is to advocate for OHP members and ensure they get quality health care.
Medicare – Senior Health Insurance Benefits Assistance (SHIBA)
The SHIBA program uses trained counselors to educate and advocate for Oregonians with Medicare.

Primary Care
Finding the Right Oregon Health Plan (OHP) Medicaid Provider
Primary Care Providers (PCP), dental providers, and behavioral health providers are essential partners in managing health. They serve as the initial contact for medical, dental, or behavioral health needs (except in emergencies), manage overall health services and treatments, and coordinate any necessary specialty or hospital care. To find a PCP, PCD, or behavioral health provider, please visit the Oregon Health Plan’s website for more information.
Community Health Centers
Oregon has 34 Federally Qualified Health Centers (FQHCs) that serve medically underserved areas and populations. They provide primary care services on a sliding scale fee based on one’s ability to pay with many takingOHP insurance. Find a list of local FQHCs here.
Behavioral Health
Behavioral Health Call Center (“Crisis Line”)
The Multnomah County Behavioral Health Call Center offers 24/7 support for individuals experiencing distress or those supporting someone in distress. Trained clinicians are available to provide assistance, resources, and referrals. Services include referrals to low-cost or sliding-scale agencies, help locating local behavioral health supports and providers, information on non-crisis community resources, and the dispatch of mobile crisis services (Portland Street Response and Project Respond).

Cascadia Walk-In Center
The Cascadia Urgent Walk-In Clinic, open 7 days a week provides urgent behavioral healthcare to community members in need.
Multnomah County Promoting Access to Home Team (PATH)
The PATH program helps people with unstable housing explore and access substance use treatment.
Multnomah County Harm Reduction Clinic
Multnomah County’s Public Health Department operates a Syringe Exchange Program and a Harm Reduction Clinic. Services offered include: syringe exchange, sexual health services, overdose rescue kits (naloxone and fentanyl test strips), and Buprenorphine.
Central City Concern Recovery
Central City Concern’s (CCC) Recovery Program offers a variety of recovery services, including but not limited to withdrawal management, outpatient care, medication-supported recovery, inpatient services, and peer support.
Peer Company Peer Support Services
Formally known as The Mental Health & Addiction Association of Oregon (MHAAO), The Peer Company helps individuals set and achieve their recovery goals, such as entering treatment, securing housing, or finding employment, while empowering people to take charge of their own path forward to recovery, health, and healing.

Urgent Care
Multnomah County has many urgent care clinics that provide urgent care services. Legacy Urgent Care is an option for all Oregon Health Plan members. For other urgent care options, clients should contact their insurance provider directly to confirm covered locations. For information on behavioral health crises, refer to the “Behavioral Health” section.

Health-Related Services “Flex” Funds (HRSF)
HRSF covers a range of services and items beyond standard benefits, focusing on those that directly improve health outcomes based on the member’s diagnosis and
treatment plan. These services are provided on a case-by-case basis through OHP’s contracted Coordinated Care Organizations (CCOs).
To request HRSF services, contact your primary care doctor or CCO. More information is available for Health Share and Trillium members.
Health Related Social Needs (HRSN)
HRSN is a new Medicaid benefit offering climate, housing, and nutrition support. Members may access up to six months of rental and utility assistance if they have a qualifying health condition, maintain a current lease, meet income requirements, and need support to maintain their current housing.
To apply for HRSN benefits:
